What Is Focused Shockwave Therapy?

Focused shockwave therapy is a precise, high-energy treatment that uses targeted acoustic waves to promote tissue regeneration and pain relief deep within the body. Learn more about how focused shockwave therapy works and why it might be right for you.

Unlike radial shockwave, which affects surface tissue, focused therapy penetrates specific structures such as tendons, ligaments, and deep muscle layers.

These sound waves increase blood flow, stimulate collagen production, reduce inflammation and help break down scar tissue — making it an ideal option for chronic or hard-to-treat conditions.

There are now multiple, randomised, double-blinded clinical trials that support the use of shockwave therapy for plantar fasciitis, lateral epicondylitis, and calcific tendinitis of the shoulder.

Curious about the difference between focused and radial shockwave therapy? Here’s how they compare.

Conditions We Treat with Focused Shockwave Therapy

Shockwave therapy is highly effective for treating chronic soft tissue conditions that have not responded well to rest, manual therapy, or exercise alone. Common conditions include:

  • Tennis elbow (lateral epicondylitis)

  • Golfer's elbow (medial epicondylitis)

  • Plantar fasciitis

  • Achilles tendinopathy

  • Rotator cuff tendinopathy

  • Calcific tendonitis

  • Hip and gluteal tendinopathy

  • Knee and patellar tendon issues

If you've been told you need shockwave therapy by a consultant or have tried other options without success, our team is now ready to help.

Why We Use the Piezowave2 System

At Physio Remedies, we’ve invested in the Piezowave2 because it’s widely regarded as the gold standard in focused shockwave therapy. Here’s why:

  • Pinpoint precision: We can accurately target the affected tissue, adjusting depth and energy for maximum effectiveness.

  • Improved comfort: The piezoelectric crystals allow smooth, controlled wave delivery with less surface irritation.

  • Custom settings: Tailored treatment intensity and duration to suit your condition and tolerance.

  • Clinically backed: Trusted by leading orthopaedic clinics and supported by NICE guidance for tendon issues.

The Piezowave2 allows us to treat deep and superficial structures with greater control and comfort compared to radial shockwave machines.

Is Focused Shockwave Therapy Treatment Right for You?

Shockwave therapy is suitable for many musculoskeletal conditions but may not be appropriate for everyone. We will always screen for contraindications before starting treatment.

Shockwave therapy is not suitable if you:

  • Are pregnant

  • Have blood clotting disorders or take anticoagulants

  • Have cancer or infection in or near the treatment area

  • Have had a recent steroid injection in the same site

If focused shockwave therapy isn’t right for your condition, we’ll help guide you towards the best alternative treatment.

  • Each session typically lasts 10 to 20 minutes, depending on the area and condition being treated.

  • Yes — most people can return to their normal routine straight away, though we may recommend avoiding intense physical activity for 24 to 48 hours.

  • Piezowave2 delivers focused acoustic waves into the affected area. These waves stimulate blood flow, reduce inflammation, and kickstart the body’s healing process.

  • It’s effective for tendonitis, plantar fasciitis, rotator cuff issues, muscle strains, tennis elbow, and many other types of chronic musculoskeletal pain.

  • Following a shockwave treatment your physiotherapist will advise that you do not take any anti-inflammatory medication such as Ibuprofen, as it will potentially lessen the treatment effect. Shockwave stimulates a new healing response i.e. it is a pro-inflammatory treatment and so we do not want to inhibit this process. If you feel any discomfort following treatment, we advise that you avoid activities that aggravate the pain and you can take paracetamol.
